Certified Nursing Assistant (CNA) – Home Health & Hospice
Ascend Health is looking for a compassionate and dedicated Certified Nursing Assistant (CNA) to join our Home Health and Hospice team. As a CNA with Ascend, you will play a vital role in providing hands-on care and support to patients and families in the comfort of their homes.

Key Responsibilities:
- Provide personal care, companionship, and support to patients in their homes.
- Assist with activities of daily living (bathing, dressing, grooming, feeding, mobility, toileting).
- Monitor and report patient condition changes to the supervising nurse.
- Provide emotional support to patients and families.
- Document all patient care and maintain accurate, timely records in the EMR.
- Follow the plan of care as directed by the clinical team.
Qualifications:
- Current CNA certification in the state of Idaho (or ability to obtain).
- Previous experience in home health, hospice, or patient care preferred.
- Compassionate, patient, and reliable with strong communication skills.
- Valid driver's license, reliable vehicle, and current auto insurance.
- Ability to work independently while being part of a supportive team.
