Job Overview
We are seeking a compassionate, motivated, and organized Community Integration Specialist (Adult Mental Health Case Manager) to join our team. Ideal Educational Background
Qualified candidates typically hold (or are working toward) a degree in:
- Social Work
- Psychology
- Human Services (or related field)
- Social & Behavioral Sciences
- Sociology
- Other allied behavioral health fields
**Graduate students or individuals continuing education beyond a bachelor’s degree are encouraged to apply.**
Responsibilities
- Conduct biopsychosocial assessments to identify client strengths, needs, and individualized goals.
- Provide community-based case management services in accordance with Section 17 MaineCare guidelines.
- Assist clients in accessing and coordinating services including housing, employment, medical care, benefits, and social supports.
- Support clients in developing coping skills, daily living skills, and strategies to promote recovery and independence.
- Monitor client progress and update treatment plans as needed.
- Collaborate with healthcare providers, community agencies, and natural supports.
- Maintain accurate, timely, and compliant documentation in accordance with MaineCare and confidentiality standards.
Qualifications
- Must hold MHRT-C certification or meet eligibility requirements to obtain MHRT-C (qualifying associate’s or bachelor’s degree in a related field required). Certification must be obtained within 2 months of hire.
- Valid driver’s license and reliable transportation (required for community-based work).
- Ability to work independently and manage a flexible schedule.
- Strong communication, organizational, and documentation skills.
