Job Overview

We are seeking a compassionate, motivated, and organized Community Integration Specialist (Adult Mental Health Case Manager) to join our team. In this role, you will support adults with mental health diagnoses in building skills, accessing community resources, and increasing independence in the least restrictive environment.

This is a community-based position that offers flexibility, autonomy, and the opportunity to make a meaningful impact in the lives of individuals navigating mental health challenges.

This is an excellent opportunity for recent graduates or individuals pursuing advanced education in behavioral health–related fields.

Ideal Educational Background

Qualified candidates typically hold (or are working toward) a degree in:

  • Social Work
  • Psychology
  • Human Services (or related field)
  • Social & Behavioral Sciences
  • Sociology
  • Other allied behavioral health fields

**Graduate students or individuals continuing education beyond a bachelor’s degree are encouraged to apply.**

Responsibilities

  • Conduct biopsychosocial assessments to identify client strengths, needs, and individualized goals.
  • Provide community-based case management services in accordance with Section 17 MaineCare guidelines.
  • Assist clients in accessing and coordinating services including housing, employment, medical care, benefits, and social supports.
  • Support clients in developing coping skills, daily living skills, and strategies to promote recovery and independence.
  • Monitor client progress and update treatment plans as needed.
  • Collaborate with healthcare providers, community agencies, and natural supports.
  • Maintain accurate, timely, and compliant documentation in accordance with MaineCare and confidentiality standards.

Qualifications

  • Must hold MHRT-C certification or meet eligibility requirements to obtain MHRT-C (qualifying associate’s or bachelor’s degree in a related field required). Certification must be obtained within 2 months of hire.
  • Valid driver’s license and reliable transportation (required for community-based work).
  • Ability to work independently and manage a flexible schedule.
  • Strong communication, organizational, and documentation skills.
