Position Overview
The Patient Care Coordinator (PCC) at ASAP Results serves as the main link between patients, healthcare providers, and internal teams. This position ensures each patient encounter—from scheduling and collection to results follow-up—is handled with accuracy, professionalism, and empathy. The PCC supports operational flow, maintains HIPAA compliance, and contributes to the company’s mission of delivering efficient, accurate, and compassionate healthcare solutions.
Key Responsibilities
Patient Coordination & Support
- Schedule and confirm patient appointments for specimen collection (UTI, STI, Nail Fungal, PGx, and other diagnostic tests).
- Provide clear collection instructions and ensure patient understanding.
- Greet and support patients with professionalism and compassion.
- Follow up with patients regarding specimen collection and results.
Documentation & Compliance
- Verify patient identity and obtain proper consent.
- Maintain complete and accurate documentation in accordance with HIPAA.
- Ensure all chain-of-custody and requisition forms are completed accurately.
- Maintain an error rate of 2% or less on documentation.
Operational & Team Support
- Coordinate with Territory Managers, Logistics, and Laboratory teams to ensure smooth operations.
- Manage daily patient scheduling logs and communicate with team via Connecteam platform.
- Track specimen status, supply usage, and ensure timely delivery to labs.
- Identify and report service gaps or operational issues.
Customer Service & Communication
- Respond to patient inquiries and escalate concerns when necessary.
- Serve as the liaison between patients, healthcare providers, and internal departments.
- Maintain patient satisfaction levels above 90%.
Qualifications
- High school diploma or equivalent required; associate degree or healthcare certification preferred.
- Minimum 1 year of experience in a healthcare, lab, or patient-facing role.
- Strong communication, empathy, and multitasking skills.
- Proficiency in Google Workspace, Connecteam, or EHR/LIMS systems.
- Ability to maintain confidentiality and professionalism.
Performance Expectations
- Maintain an error rate of 2% or less in documentation and patient tracking.
- Ensure patient scheduling goals and follow-up rates are met weekly.
- Uphold company compliance and quality assurance standards.
- Participate in audits and ongoing training.
Pay: $20.50 - $24.03 per hour
