Position Summary
The Nurse Practitioner provides professional health services through assessment, diagnosis, education and/or treatment for acute and chronic health problems along with preventive care and/or palliative/hospice. This position specifically provides anticoagulation therapy with warfarin (or Coumadin), integrating INR result monitoring, doing algorithms and assessment based on the Antithrombotic Therapy and Prevention of Thrombosis through American College of Chest Physicians (ACCP) Clinical Practice Guidelines.
Qualifications
- Graduation from an accredited Nurse Practitioner Program approved by the American Medical Association required.
- NP licensed by Oregon State Board of Nursing AND RN licensed by Oregon State Board of Nursing required.
- BLS from American Heart Association (AHA) required upon start.
- ACLS required upon start.
- FNP licensed by Oregon State Board of Nursing preferred.
