The RN Discharge Coordinator/Case Manager assesses patients for transitional care needs and barriers to discharge, communicates with patient and family regarding preferences, charts and documents the plan of care, and coordinates discharge plans involving community resources and alternate levels of care. Facilitates successful adherence to the discharge plan.
Qualifications: BSN or bachelor degree in allied health preferred. 2 years progressive nursing in acute care with focus on discharge, utilization, or case management. RN by OSBN required. CCM certification preferred.
Salary: $49.33-$67.84/hr. On Call.
