Family Medicine Physician – Outpatient Only
A mission-driven, team-based primary care program in Elkhart, Indiana, is seeking a Family Medicine Physician to join an outpatient-only practice serving the community through exceptional, accessible care.

Responsibilities
- Provide comprehensive preventive care and chronic disease management
- Perform minor office procedures
- Deliver women’s health services, including paps, contraception counseling, and implants
- Partner with care coordination teams to support quality and access goals
Schedule
- Monday through Friday, 8:00 AM to 6:00 PM
- 4 clinic days and 1 administrative day each week
- No weekends
Compensation & Benefits
- Base salary of $250,000 DOE
- $18,000 sign-on bonus
- Federal student loan repayment
- 4 weeks PTO plus 11 paid holidays
- CME allowance of $2,500 and 5 CME days
- FTCA malpractice coverage

- 403(b) with 3% match
