Position Name: Registered Nurse / RN - Home Health

Location: Salem, OR

Territory: Salem and Surrounding areas

Employment Type: Direct-Hire & Permanent

Setting: Home Health

Pay: $52-$62/hour, depending on experience (10k Sign on Bonus)

Shift: All Shifts Available (Full-Time, Part-Time, Per Diem, 3x12s, 4x10s, Weekends)

Qualifications:

Required:

- Active Oregon Registered Nurse license

- Associate’s or Bachelor’s degree in Nursing

- Minimum of 1 year of recent acute care nursing experience

- Current BLS and hands-on CPR certification

- Valid driver’s license with reliable, insured transportation

Preferred:

- Previous home health nursing experience

- Experience managing an assigned patient caseload

- Strong assessment, care coordination, and patient education skills

Job Summary:

- Complete comprehensive in-home nursing assessments and identify each patient’s clinical and ongoing care needs.

- Develop, implement, and update individualized plans of care in collaboration with patients, families, physicians, and the interdisciplinary team.

- Provide skilled nursing care, education, and support to patients and caregivers in the home setting.

- Monitor changes in patient condition and communicate updates or concerns to the appropriate members of the care team.

- Coordinate services and assist with discharge planning to support safe transitions throughout the patient’s episode of care.

- Maintain accurate and timely clinical documentation while independently managing an assigned territory and caseload.
