Job Title: Case Manager (Community Support Team)
Home Office: Vienna, IL; Anna, IL; or Cairo, IL. Location dependent on closest to home residence and availability.
Area of Focus: Behavioral Health
Program(s): Behavioral Health: Outpatient Mental Health Programming helps individuals build life skills and well-being through behavioral health counseling, psychiatric care, and other care coordination and support services. SAMHSA CCBHC: Serves individuals living with a mental or substance use disorder who seeks care, including those with severe mental illness (SMI), substance use disorder (SUD) including opioid use, children and adolescents with serious emotional disturbance (SED), individuals with co-occurring mental and substance disorders (COD), and individuals experiencing a mental health or substance-related crisis.
Immediate Supervisor: Programs Coordinator
Case Manager Hours of Work: Full-time, 40 hours/week. Afterhours on-call rotation required.
Case Manager Minimum Qualifications: Bachelor’s Degree in Human Services or related field must qualify as a Mental Health Professional (MHP).
Case Manager Skills & Knowledge: Knowledge of behavioral and human development theories and practices; skilled in the delivery of case management/care coordination services, knowledge of local health and human services resources, conflict resolution, excellent written and oral communication skills, excellent computer skills; knowledge of basic crisis intervention and conflict resolution techniques.
Case Manager Special Requirements: Travels to off-site/community-based locations for direct service delivery; participates in after-hours crisis response rotation; ability to schedule evening appointments; maintains and provides proof of a valid driver's license; maintains and provides proof of personal vehicle liability insurance; at least 21 years of age; maintain First Aid & CPR certification; must complete required registry clearances and criminal fingerprint background checks; use personal vehicle when agency vehicle is not available.
Case Manager job duties include, but are not limited to:
- Provides comprehensive consumer care and case management services including access to transportation, state and federal entitlements, medical care, psychiatric care, housing, and other basic needs to support stabilization and achieve/maintain recovery.
- Provide community-based support to build on independent activities of daily living such as grocery shopping, attending medical appointments, assisting with medication reminders, social skills training and development, money management and budgeting skills, and coping skills development.
- Works collaboratively with the behavioral health treatment team to identify program goals and monitor progress in treatment; intervenes by way of conducting activities such as skill building, advocacy, recovery management planning, and support and consultation to improve the overall health and well-being of the client served and overall level of functioning in identified areas of life.
- Facilitates communication across agency staff and applicable programs to ensure continuity of care and timely follow-up for individuals accessing behavioral health services.
- Assists as needed with setting up transitions in care and appropriate linkages for those entering or exiting higher level of care.
- Teaches life-management and daily living skills to clients that translate to their natural environment with an emphasis on developing natural supports.
- Completes direct service billing requirements in an efficient and timely manner as defined on the Direct Service Requirement form.
- Completes all required forms and documentation in a timely and efficient manner.
- Demonstrates active involvement in team/program meetings, supervision, and agency/community events and activities as assigned.
- Provides crisis response and intervention services to clients by utilizing short-term crisis intervention and conflict resolution techniques and referral to appropriate health and human service resources.
