Case Managers assist individuals with intellectual and developmental
disabilities in identifying and coordinating the use of both paid
services and natural supports available to enhance the persons
independence and community participation based on the persons
capabilities and preferences.

Responsibilities include building upon assessment information to develop
an individuals person-centered support plan to assist the person in
meeting their needs, personal goals and achieving their best possible
life. Case Managers provide advocacy, including pursuing means for
gaining access to needed services and entitlements, and securing
supports outlined in the person-centered support plan.
