Overview
We are seeking a passionate and dedicated Memory Care Program Coordinator to lead and enhance our specialized engagement programs for our Community Members living with dementia. This role offers an exciting opportunity to make a meaningful difference in the lives of older adults by developing and implementing, innovative programs tailored to individual needs. The ideal candidate will bring a strong background in dementia programming and a compassionate person centered approach.
This role integrates creativity, dementia expertise, and compassion to promote meaningful engagement, dignity, and well-being for each resident. The Program Coordinator develops programs that both honor residents’ life histories, interests, and utilize their remaining skills and abilities, all while fostering community connection, purpose, and joy.
Key Responsibilities
Program Development & Delivery
- Plan, organize, and lead daily, weekly, and monthly life enrichment activities tailored to residents’ cognitive, physical, social, spiritual, and emotional needs.
- Incorporate evidence-based dementia care approaches into programming (e.g., reminiscence therapy, music therapy, sensory activities, and the creative arts).
- Ensure activities are adaptable to different stages of dementia and promote inclusion for all residents.
- Maintain monthly activity calendars that are creative, balanced, and resident-driven and that comply with California RCFE (Title 22) activity requirements
Resident-Centered Engagement
- Develop individualized activity plans that reflect residents’ personal histories, preferences, and strengths.
- Collaborate with care team members and families to gather life stories and integrate them into programming.
- Create positive, structured routines that reduce agitation and enhance well-being.
- Support residents during group and one-on-one activities with encouragement, patience, and empathy.
- Maintain a strong biophilic focus to the program with the development and execution of unique daily offerings of gardening and outdoor programming.
- Utilize available technology to provide enhanced program offerings.
Collaboration & Leadership
- Partner with clinical and dining leadership and care partners to integrate meaningful engagement throughout the day.
- Train and support staff in person-centered, dementia-sensitive approaches to resident interaction and providing engagement.
- Recruit, orient, and supervise volunteers and greater community program partners (e.g., musicians, pet therapy, community groups).
Documentation & Compliance
- Maintain individualized activity documentation and ensure compliance with California RCFE (Title 22) activity requirements.
- Work with MD Senior Wellness leadership to gather and analyze program outcomes.
- Maintain safe, organized activity supplies and spaces.
Community Building
- Foster a warm, inclusive community environment where residents, families, and staff feel engaged and supported.
- Plan special events, holiday celebrations, and family engagement programs.
- Build partnerships with local organizations, schools, and cultural groups to enrich resident life.
Qualifications
- Associate’s or Bachelor’s degree in Gerontology, Recreation Therapy, Social Work, or related field preferred.
- Minimum 2 years of experience in life enrichment/activity coordination, preferably in memory care or dementia services.
- Strong knowledge of dementia, person-centered care principles, and evidence-based engagement strategies.
- Excellent communication, creativity, and organizational skills.
- Ability to lead groups, adapt activities to varying abilities, and connect meaningfully with individuals.
- Proficiency in Microsoft Office and ability to design activity calendars/flyers.
- Must meet California RCFE training requirements for activities staff.
- Must have valid CA Driver’s License.
Physical Requirements
- Ability to stand and walk for extended periods.
- Lift up to 25 pounds (activity supplies, equipment).
- Frequent bending, reaching, and movement associated with leading programs.
- Flexibility to work evenings, weekends, and holidays for special events.
