Duties
- Conduct comprehensive patient assessments to determine eligibility and develop individualized care plans that address physical, emotional, and spiritual needs.
- Manage and coordinate patient care across various settings including inpatient units, outpatient clinics, and post-acute care facilities.
- Collaborate closely with physicians, nurses, social workers, and other healthcare professionals to ensure seamless delivery of hospice services.
- Oversee discharge planning and facilitate smooth transitions between care settings while maintaining compliance with HIPAA regulations and medical records standards.
- Perform regular patient visits to monitor progress, evaluate symptoms such as vital signs and physiological responses, and adjust care plans accordingly.
- Engage in diagnostic evaluations and utilization management to optimize patient outcomes and resource utilization.
- Provide education to patients and families about disease progression, symptom management, and available support services.
Experience
- Extensive background in hospital medicine, medical-surgical units, or post-acute care facilities with a focus on patient service and assessment.
- Strong knowledge of physiology, medical terminology, discharge planning processes, and inpatient/outpatient care workflows.
- Demonstrated expertise in case management within hospital or managed care settings with an emphasis on utilization management and diagnostic evaluation.
- Experience handling complex cases involving trauma or emergency room scenarios is advantageous.
