Arkansas Provider Coalition (APC) provides Care Coordination for Summit Community Care, helping individuals with developmental disabilities and/or behavioral health diagnoses maintain their health and independence. Working with Arkansas Medicaid, APC ensures patient-centered care through service referrals, follow-ups, and navigation. The care process includes assessment, care planning, medication management, service coordination, and monitoring adherence to ensure timely access to quality, cost-effective care. Position Summary
Develops, implements, and monitors the Person-Centered Service Plan (PCSP) for assigned members designated as “complex cases” due to rapid hospital readmissions, guardian changes, juvenile system intervention, and various populations of members. These populations include, but are not limited to, Tier IV, Therapeutic Communities, Re-Entry Population, DCFS, and members on court orders for treatment. These members may have behavioral health needs, developmental/intellectual disabilities, substance use disorders, and other special conditions. The Enhanced Care Coordinator works closely with members, their families/guardians, their providers, and other members of their support circle to ensure the member receives the most appropriate care while addressing barriers to healthcare access.
Key Responsibilities

Work directly with members and their support circles to meet health and service needs.

Regularly communicate with members, families, providers, and stakeholders through various methods.

Assist with cases not yet assigned to ensure seamless care transitions.

Provide necessary information and escalate risks to implement PCSP effectively.

Conduct weekly calls with care teams and stakeholders to coordinate care.

Engage in discussions and meetings about assigned members.

Create materials to improve care efficiency and reduce risks.

Schedule and manage appointments and travel across Arkansas.

Maintain compliance with relevant regulations and ensure data confidentiality.

Maintain a home office with reliable internet and secure company assets.
Qualifications

Education: Minimum completion of 2 years of college-level coursework in social work, psychology, sociology, another social science, or a health-related field. Bachelor’s degree in social work, psychology, sociology, another social science, or a health-related field preferred.

Experience: Minimum of 2 years of experience in care coordination and at least 1 year of beginner-level experience with Microsoft Excel, Outlook, and Word. Ideally, applicants will have 3-5 years of experience in care coordination within managed care and/or the Arkansas PASSE (Provider-Led Arkansas Shared Savings Entity). Preferred qualifications include having experience with DCFS and holding either a Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) certification.

Skills: Strong communication skills in presenting both orally and in writing to individuals from diverse backgrounds and with different learning styles. Skill in mediation techniques and meeting facilitation strategies.

Hiring Hourly Range: $23.77 – $26.41 hourly. The final offer will be determined based on the candidate’s level of education, relevant experience, and overall fit for the role.
