Job Summary

This position performs case management and supportive assistance in conjunction with the Statewide division of Developmental Disabilities (DDD) Coordinator. Activities and duties pertaining to children involved with the Department of Child Safety (DCS), DDD, Arizona Early Intervention Program (AZEIP), and Arizona Long Term Care (ALTCS). Provides assistance when barriers are identified.

Job Duties

Performs care management and placement activities for children involved with DCS, DDD, AzEIP and ALTCS (DDD and EPD), who have complex healthcare (developmental and medical) presentations. Leads efforts to garner appropriate care giver to child match and ensure necessary support is in place to meet the needs of the child and caregiver. Document efforts. Requires working with children to perform the LET (Likely Eligible Tool).

Supports DCS Specialists and other stakeholders navigate the DDD determination eligibility process, including the tracking of referral, assists DCS Specialist and Placement Administration to garner necessary medical documentation from health care professionals and other required information when needed, tracks and reconciles enrollment of children and youth determined eligible for DDD services whether DD-Only, Targeted Support Coordination or ALTCS.

Submitting ALTCS applications and scheduling ALTCS interviews to help ensure all required information is accurately provided during interviews and assessments. Supporting communication between evaluators, caregivers, and DCS staff throughout the evaluation process.

Develops and maintains professional relationships among system stakeholders including DCS Comprehensive Health Plan (CHP), System of Care staff with efforts focused toward identifying child-specific and system oriented enhancements to improve health care to children served by DCS.

Participates in DCS performance management activities that support positive health care and individualized plan outcomes for children and youth. Maintains and/or manages health care and child-welfare placement data, develops and distributes reports as appropriate to staff and leadership.

Maintains up to date knowledge of DCS, DCS CHP and Department of Economic Security DDD and AzEIP policies, and applicable state laws.

Participates in activities beyond regular work hours on a rotational basis, handle incoming email and telephonic inquiries, works with providers, caregivers, DCS staff, and Mercy Care DCS Comprehensive Health Program (CHP) staff.

Selective Preference(s)

Preferred bachelor's degree in Social Work, Psychology, Special Education, plus three years of related professional experience involving program planning or administration.

Pre-Employment Requirements

  • Must be able to obtain and maintain a valid Arizona Department of Public Safety Level One Fingerprint Clearance Card
  • Must be able to secure and maintain clearance from the DCS Central Registry.
  • Employees may be required to use their own transportation as well as maintain valid motor vehicle insurance and current Arizona vehicle registration; however, mileage will be reimbursed.
  • This position requires three years of professional experience related to health programs appropriate to the assignment, which included program planning or administration; a Master's degree in a field appropriate to the assignment may substitute for one year of the required experience.

This position requires driving or the use of a vehicle as an essential function of the job to conduct State business, and the following requirements apply: Driver’s License Requirements.

Bi-Lingual Stipend

In an effort to better serve our communities, DCS offers a Bi-Lingual Stipend to all interested and eligible DCS Employees who successfully complete a language proficiency exam. Qualifying employees will receive a $500 annual stipend paid in equal increments.

Positions in this classification participate in the Arizona State Retirement System (ASRS).

ASRS participation may begin immediately (if you were a previous member) or upon your 27th week of employment (if you have never been an ASRS member previously).
