Social Work Director
AristaCare at Cedar Oaks is seeking a Social Work Director to join our team. In this role, you will provide medically-related social services to attain or maintain the highest practicable physical, mental, and psycho-social well-being of each patient.
- Develop and implement case management services for all residents.
- Maintain contact with family (with resident’s permission) to report on changes in health, current goals, discharge planning, and encouragement to participate in care planning.
- Maintain progress notes under federal and state guidelines for Long Term Care.
- Facilitate the building of relationships between residents and staff by teaching staff how to understand and support residents’ individual needs.
Requirements:
- BA from an approved Social Work school is required.
- Subacute and geriatric experience
- Minimum one year's experience with seniors. In a Supervisory capacity in a hospital, skilled nursing care facility, or other related medical facilities.
- Must have a valid CSW or LSW license in New Jersey
