What You'll Do:
- Provide in-home assessments and develop personalized treatment plans
- Deliver hands-on care including orthopedic, trigger point, and manual therapy
- Educate patients and families on rehab strategies and home exercises
- Coordinate with nurses, case managers, and other providers for continuity of care
- Maintain timely and accurate clinical documentation
- Support discharge planning and long-term mobility goals
What We’re Looking For:
- Licensed Physical Therapist in the state of AZ
- Home health experience is a plus—but not required
- Strong communication and self-management skills
- Reliable transportation and valid driver’s license
- Compassionate, professional, and patient-centered approach
