Job Summary
We are seeking a dedicated and compassionate Advanced Practice Registered Nurse to join our healthcare team. The ideal candidate will have experience in both inpatient and outpatient settings, with a strong focus on patient assessment and care for individuals with developmental disabilities. This role requires a commitment to providing high-quality medical care, conducting clinical research, and managing pain effectively. The Advanced Practice Registered Nurse should have a strong understanding of societies and their impact on mental health; ability to solve problems, monitor and examine patients.

  • Possess bachelor's degree in nursing
  • Master's degree in nursing from a state approved and accredited school for professional nursing training
  • Possess APRN License
  • Possess and maintain graduate professional nurse, and nurse practitioner registration in The Commonwealth of Indiana, which must be unrestricted, current, complete, and active
  • Possess and maintain an unrestricted DEA license and hold or be willing to obtain an X-Waiver to prescribe medication to treat substance use disorder clients
  • Must be willing to sign collaboration agreement with medical director for X-waiver management
  • Must be willing to sign a non-compete agreement

Skill Sets: Proficient in the English language, both in writing and speaking to be able to communicate clearly with all stakeholders, including patients, families, staff, and other healthcare professionals. Maintain a supportive and encouraging working relationship with supervisor, leaders, and clients. Must have basic computer skills, communication skills and demonstrate a non-judgmental and non-bias disposition with regard to clients. Be compassionate, sensitive, and possess strong reasoning and thinking skills. Possess strong computer skills, including working with word processing and data entering applications, as well as with medical software applications. Proficient in lab/EKG interpretation and strong diagnostic reasoning and assessment skills.

Physical and Environmental Demands: Visual acuity and manual dexterity are needed for required paperwork. Frequent talking and hearing are required when communicating with leaders, families, and other service providers. Physical demands are moderate with walking, standing, and stooping with frequent bending, stretching twisting, and reaching out. Possible exposure to communicable diseases during contact with individuals released from various locations i.e., jails, hospitals, unsanitary conditions, rehabilitation centers, etc. Travel between sites required.

Job Responsibilities:

1. Responsible for providing care to clients with poly-substance abuse disorders that may have chronic or urgent medical and mental health conditions.

2. Complete initial medical and mental health assessments with all new clients in residential settings.

3. Perform client’s histories and physicals, risk factor assessment, ordering of therapeutic and diagnostic tests; and attend to requested consultations and appointments.

4. Conduct follow-up visits with clients per company protocol and provide ongoing care and monitoring to clients while in residential setting.

5. Determine appropriate treatment plans for clients in acute stages of detox from illicit substances and monitor staff compliance with service delivery and client’s progress through detoxification stage.

6. Provide onsite guidance to other licensed medical staff at residential settings and collaborate with non-licensed staff.

7. Work independently with other members of the medical team, with entitlements to specific privileges approved by the Medical Director and Director of Medical and Nursing Services.

8. Review new intake screenings in a timely manner and determine if potential intakes are medically appropriate for residential level of care.

9. Participate in a rotating on-call schedule and provide remote guidance to residential staff during on-call shifts.

10. Take part in an interdisciplinary team that provides holistic, person-centered care to clients.

11. Prescribe medications following the company policy, formulary and relevant regulations of the State of Indiana.

12. Consult with Director of Medical Services and if case is beyond the scope of qualification of APRN, consult with physician/MD (Medical Director).

13. Enter all relevant client data in medical records in the facilities electronic medical records information management system in a timely manner. Ensure records are detailed, comprehensive, factual, and legible.

15. Monitor client progress through program by reviewing COWS/CIWA scores, daily vital signs, compliance with medications, group attendance, and symptomatic complaints. Detail progress in client medical record in the electronic medical record management system.

16. Formulate, execute, and review healthcare education delivered to clients for disease management, relapse prevention, and treatment planning.

17. Ensure smooth communication with other health professionals for effective delivery of service to patients.

18. Provide consultation to Medical Director regarding identified areas of quality improvement and take active role in enhancing overall quality of services delivered to clients.

19. Consult and collaborate with Program Directors/Program Managers/Case Managers to coordinate client progression through residential program.

20. Actively engage clients in relapse prevention planning, provide guidance on available options, and initiate appropriate plans based upon company policy.

Job Type: Full-time

Pay: $90,000.00 - $130,000.00 per year
