Responsibilities:
- Provide direct patient care in home settings, including assessments, treatments, and medication administration
- Develop, implement, and monitor individualized patient care plans
- Perform wound care, IV therapy, catheter care, and other skilled nursing procedures as needed
- Monitor and document patient health status, progress, and changes in condition
- Communicate effectively with physicians, care coordinators, and family members regarding patient care
- Educate patients and caregivers on disease management, medications, and home safety
- Maintain accurate and timely clinical documentation in compliance with state and federal regulations
Requirements:
- Active RN or LPN license in the state of Ohio in good standing
- Previous home health, long-term care, or clinical nursing experience preferred
- Strong assessment, critical thinking, and clinical decision-making skills
- Excellent communication and interpersonal skills with patients and families
- Reliable transportation and valid driver's license for home visits throughout Columbus, OH
- Ability to work independently with minimal supervision
- CPR/BLS certification required or ability to obtain upon hire
