Job Summary
We are seeking a dedicated and compassionate Case Manager to join our outpatient mental health team. In this role, you will support clients by coordinating individualized care plans, connecting them with community resources, monitoring treatment progress, and collaborating with therapists, psychiatric providers, families, and other community partners to promote continuity of care. You will play a key role in helping clients overcome barriers to treatment, develop independent living skills, and achieve their recovery and wellness goals. Duties
- Conduct comprehensive client assessments to identify behavioral health needs, treatment goals, strengths, barriers, and community resource needs.
- Develop and maintain individualized service plans in collaboration with clients and the treatment team to support recovery and overall wellness.
- Coordinate care with therapists, psychiatric providers, primary care providers, families, referral sources, and community agencies to ensure continuity of care.
- Assist clients with accessing community resources, including housing, employment, education, transportation, benefits, and other supportive services.
- Monitor client progress, maintain regular contact, and provide ongoing case management services to promote treatment engagement and goal achievement.
- Facilitate referrals and coordinate transitions of care to appropriate levels of behavioral health and community-based services.
- Maintain accurate, timely, and thorough documentation in the electronic health record while adhering to confidentiality and privacy requirements.
- Participate in multidisciplinary treatment team meetings and contribute to treatment planning and care coordination.
- Conduct psychoeducational and process groups several times each week, utilizing evidence-based interventions to support client recovery, skill development, and emotional well-being.
- Maintain knowledge of community resources and advocate on behalf of clients to reduce barriers to treatment and improve access to services.
Requirements
- Certified Alcohol and Drug Counselor (CADC) or equivalent certification preferred.
- Previous experience in behavioral health, mental health, substance use treatment, case management, or a related human services setting required.
- Knowledge of community resources, care coordination, crisis intervention, and recovery-oriented practices.
- Familiarity with FMLA, California State Disability Insurance (SDI), and EDD processes, including assisting clients with required documentation and referrals.
- Ability to develop individualized service plans, coordinate care, and advocate effectively for clients.
- Experience facilitating psychoeducational or process groups, or willingness to lead groups several times per week.
- Strong organizational, time management, and documentation skills with the ability to manage multiple clients and priorities.
- Proficiency with electronic health record (EHR/EMR) systems and Microsoft Office applications.
- Knowledge of confidentiality requirements, including HIPAA, and applicable behavioral health regulations.
- Excellent interpersonal and communication skills with the ability to collaborate effectively with multidisciplinary treatment teams, referral sources, clients, and families.
- Ability to work independently while maintaining a client-centered, compassionate, and professional approach to care.
