The basic responsibilities of a Hospice Registered Nurse/Case Manager (RN/CM) are defined by the CMS Conditions of Participation (42 CFR Part 418), hospice standards, and OUR accrediting organization, The Joint Commission. The RN/CM serves as the primary coordinator of the patient's hospice care and ensures that the patient's physical, emotional, psychosocial, and spiritual needs are addressed through the interdisciplinary team (IDG).
The Hospice RN/CM serves as the clinical leader and coordinator for each assigned patient's care. Their primary responsibilities include assessing the patient, managing symptoms, coordinating services, educating and supporting the patient and family, collaborating with the interdisciplinary team, maintaining regulatory compliance, and ensuring that hospice care remains focused on comfort, dignity, and the patient's goals throughout the end-of-life journey.

Pay: $54.00 - $56.00 per hour
