We're seeking a Hospice Quality Manager who brings a deep understanding of hospice care, a commitment to excellence, and a passion for ensuring every patient receives compassionate, compliant, high-quality service.
This is a hands-on leadership role preferably a Licensed Nurse (LPN) who thrives on driving performance improvement, supporting clinical teams, and ensuring survey readiness while keeping patients' comfort and dignity at the center of everything we do.
What You'll Do:
- Oversee and enhance the quality of care across hospice services through regular audits, data analysis, and collaboration with clinical teams.
- Lead and facilitate QAPI meetings at least quarterly, summarizing the QAPI Program to include logs (Grievance, Problem-Solver, Incident, Infection Control, and Hospitalization, etc.) survey readiness efforts, and follow-up plans of correction when necessary.
- Conduct focused clinical audits to identify opportunities for improvement and celebrate best practices.
- Deliver training and guidance to field staff, ensuring adherence to hospice regulations and standards of care.
- Analyze quality metrics and report outcomes that drive meaningful changes in patient and family experiences.
· Ensure timely and accurate submission of reports to relevant regulatory bodies. Stay up to date with changes in regulations and ensure the organization's compliance.
- Work closely with the Nursing Director and the Psych-social Director to ensure coordinated and comprehensive patient care.
- Lead and participate in quality assurance projects aimed at improving patient outcomes and operational efficiency.
- Assists with orientation and on-going training of all employees regarding documentation.
- Reviews all reports of complaints, errors, incidents, and grievances to ensure that they are fully documented and properly handled in the time-frame and manner specified by Agency policy.
- Performs other duties as assigned by the supervisor.
Desired Skills Qualifications
- 1 year of home health, hospice, or other medical related experience preferred
- Licensed Vocational Nurse or Licensed Practical Nurse with two years of QI (quality improvement) experience preferred.
- Current and valid Nursing license in the state(s) practicing.
- Graduation from an accredited School of Practical/Vocational Nursing.
- Strong working knowledge of hospice regulatory and compliance requirements.
- Knowledgeable of Medicare and Medicaid guidelines.
- Must have excellent written communication by writing clearly and informatively, edits work and able to read and interpret legal written information.
- Must practice a high level of confidentiality and be able to effectively response to questions from employees, managers and outside entities.
- Good communication skills and ability to establish good rapport with other staff members.

- Ability to work independently and complete tasks timely
- Proven ability to analyze data, lead quality initiatives, and support performance improvement.
- Adaptive problem-solving skills
- Must have knowledge and proficiency of office computer equipment and software. Demonstrate ability to multitask and work in a fast-paced office setting. Proven ability to cope with conflict, stress and crisis situations.
This position requires at least the following minimum requirements:
- Current and valid Nursing license in the state(s) practicing.
- Graduation from an accredited School of Practical/Vocational Nursing.
- Reliable transportation and a valid driver's license.
- Valid AZ Fingerprint Clearance Card (we can help)
- Current CPR Certification (we can help) Negative TB Test (we can test)

Pay: $65, $72,000.00 per year
