    PROGRAM: Safe Options Support (SOS) –Team

 

    REPORTS TO: CBC Program Director 

 

      Job summary: The Overnight SOS Team Leader plays a critical role in providing outreach, engagement, and support to street homeless individuals during overnight hours. They lead a team of dedicated professionals and work closely with community partners to connect individuals with shelter or emergency housing options. The Team Leader ensures effective communication, handoffs, and coordination with day teams to provide seamless and continuous care. The SOS team will consist of Team leader, Licensed Clinician, Care Managers, and Peer Specialist. The team will outreach and engage street homeless individuals in the community to provide intensive outreach, engagement, and care coordination services. This role serves as a liaison with the lead contracting entity (CBC) and participates in policy and quality management workgroups. The role will require field-based work, on call coverage, and a willingness to work flexible hours.


Minimum Education and Experience Requirements:  Licensed Master’s degree or higher in Social Work, Mental Health Counseling, Public Administration, Nursing, Public Health, Public Policy or a related field. A minimum of 3 to 5 years post-masters work experience in the behavioral health and criminal justice field with progressively increasing responsibilities, preferably with the target population. 

 

     Essential Duties and Responsibilities

  • Management and oversight of the day-to-day activities of the Safe Options Support team in accordance with internal and external regulatory standards and guidelines;
  • Coordinate team schedules and ensure sufficient coverage during overnight hours to meet program needs.
  • Lead the overnight SOS team in conducting outreach to locate and engage street homeless individuals in need of immediate assistance.
  • Build rapport and establish trusting relationships with individuals experiencing homelessness, providing support, empathy, and guidance.
  • Connect individuals with available shelter or emergency housing resources, ensuring their safety and immediate needs are addressed.
  • Collaborate with community partners, including shelters and housing providers, to facilitate access to appropriate services and resources.
  • Engagement beginning either at known “hang-outs” or “Hot spots” within the transit system or during an inpatient hospital admission or emergency department visit with involvement in transition planning, including a needs assessment of community transition supports essential to stabilizing the participant;
  • Outreach to communities, Street Outreach Teams, police, hospitals, providers, and community/family members and other caregivers to help identify individuals who would benefit from referral to a SOS CTI Team
  • Participate in case conferences to help identify community health needs and tailor services to meet these needs;
  • Ensures that programmatic systems are in place and functioning to meet participant needs;
  • Ensures that internal and external regulatory agency standards and requirements are met;
  • Provide regular individual and group supervisory support to a multi-disciplinary team and orienting new team members to policy and procedures as well as ongoing training and mentorship;
  • Assists with the identification, development, implementation, and completion of quality improvement projects and plans to solve problems/issues; 
  • Facilitate and monitor the organizing of staff schedule; as well as leading weekly staff meetings;
  • Perform chart reviews and ensure completion of appropriate and timely documentation within Health Information Technology system; 
  • Complete assessments and service plans as needed;
  • Collect, analyze and report data as required by the agency and CBC; 
  • Develop relationships with providers to facilitate successful transition for participants and to expedite and solidity placement in community housing;
  • Mange teams on-call duties and ensure 24/7 crisis response availability and provide crisis intervention services on a rotating basis, including referrals to respite and other resources;
  • Respond and liaison between requests from SOS on Call line and individuals in need
  • Manage recruitment of staff, as well as addressing staff performance-related issues in accordance with salient policies and procedures, specific to the organization of hire; 
  • Screen candidates for program and determine eligibility. Assign caseloads to team members;
  • Work with all stakeholders (members, network, families, and government staff) in a caring and respectful manner, and with due understanding of and consideration for cultural differences;
  • Maintain communication with and act as key point of contact with CBC staff;

 

    Qualifications:

  • Flexibility to work overnight hours, including weekends and holidays, as needed to ensure 24-hour coverage and support for the SOS program.
  • Knowledge of mental illness and serious emotional disturbances and substance use disorders;
  • In depth knowledge of NYC behavioral health and community support programs and systems.
  • Knowledge of homeless resources, NYC shelter systems, and MTA transit systems.
  • Experience working with homeless and precariously housed populations.
  • Knowledge of treatment, rehabilitation, and community support programs as they relate to consumers/residents, families, and staff;
  • Knowledge of multi-disciplinary team experience, preferred;
  • Knowledge of techniques for identifying and preventing potentially violent behavior, including crisis management techniques;
  • Ability to develop, evaluate, implement and modify a clinical treatment intervention to meet the needs of individual participants;
  • Ability to prepare accurate and timely reports;
  • Ability to manage multiple projects and ask for help when needed;
  • Computer proficiency in Electronic Health Records and Microsoft applications such as MS Word, Excel, PowerPoint
  • Ability to utilize strength-based and recovery-oriented approaches (I.eHarm reduction, motivational interviewing, trauma informed care)
  • Ability to communicate effectively with stakeholders;
  • Competency in written, interpersonal, verbal and computational skills to present and document records in accordance with program standards;
  • Preferred familiarity with systems and tools commonly utilized across HH programs (i.e MAPP, UAS, EMEDNY, PSYCKES).
  • Available to work a flexible schedule in response to participant and staff needs;


Employment is contingent upon the successful completion of a background check and/or fingerprinting (department-dependent), which will be conducted following a conditional offer of employment in adherence to NYC Fair Chance Act guidelines.

Salary : $90,000
