Job Title: Community Outreach Worker/Driver North Bronx

Job Summary: The Community Health Outreach Worker North Bronx provides mobile outreach services in the North Bronx and other to provide health education, risk reduction information and safe injection equipment and other supplies to people who use drugs. Conduct Harm Reduction Services and Syringe exchange program enrollments, including review of substance use history, current drug use practices, medical waste disposal collection and practices, and providing low-threshold counseling and education to participants within the agency and on the mobile unit
Primary Duties & Responsibilities include, but are not limited to:
• Ability to operate a motor-vehicle (vans of various sizes)
• Conduct Harm Reduction Services (HRS) and syringe services program enrollment
• Serve as agency liaison at community forums, events, and planning bodies as appropriate and where assigned
• Data collection is required
• Conduct Opioid Overdose Reversal Training
• Designated Opioid Overdose Responder
• Ability to explain, describe, and discuss safer injection practices, other safe drug using strategies, and safer sex options
• Provision of and documentation of prevention education, counseling, referrals, safer drug using education, and strategies
• Street level outreach for program recruitment and retention, including distribution of health education and risk reduction information
• Act as a point person for Buprenorphine referrals and information
• Ability to train to conduct rapid HIV and Hepatitis C on the mobile unit
• Responsible for ensuring medical waste is handled properly, packed, and stored in a designated area
• Preparation, packing, and inventory of syringes and related program supplies
• Provision of and documentation of prevention education, counseling, referrals, safer drug using education, and strategies
• Ability to distribute high-risk prevention supplies, including female & male condoms, safer smoking kits, and provision of sterile syringes
• Monitoring the inventory, stocking and ordering of harm reduction supplies
Qualifications/Criteria:
• Driving Experience: Candidates must possess substantial experience driving large vehicles, such as large vans or trucks. Experience in maneuvering these vehicles safely in various driving conditions is essential.
• Driver’s License Compliance: It is required that your driver’s license remains in good standing at all times. A valid New York State driver’s license is mandatory, with a minimum of two years of professional driving experience.
• Flexible Availability: The role demands availability to work flexible hours, which may include evenings, and weekends.
• Commitment to Professional Development: We prioritize ongoing professional development. The ideal candidate will actively engage in all required training sessions, webinars, and additional learning opportunities.
• Experience in Harm Reduction Counseling: A minimum of two years of demonstrated experience in providing harm reduction or substance use counseling is required. This includes offering referrals, prevention education, and safer substance use strategies. Familiarity with community resources and the ability to connect clients to these services is crucial.
• Prompt Incident Notification: You are required to notify the BOOM!Health Human Resources Department and your immediate supervisor immediately if any driving incident or accident occurs. Timely reporting is essential to manage the situation effectively and maintain safety protocols.
• Incident Reporting: As a representative of BOOM!Health, you will be responsible for diligently completing Incident Report Forms for any incidents or accidents that occur during mobile unit outreach activities. These reports must be submitted promptly to the Department Director and the Human Resources department to ensure proper documentation and follow-up.
• Knowledge of Infectious Diseases: Candidates should have an understanding of infectious disease transmission and prevention practices, particularly related to injectors. This includes knowledge of HIV, Hepatitis C, and Opioid Overdose Prevention and Response strategies.
• Risk Reduction Counseling: The role requires the ability to provide risk reduction and safer drug use counseling effectively. You should be able to offer appropriate referrals for care and treatment to support clients in their health and recovery journey. Experience in health education, outreach strategies, and harm reduction theory and practice will be valuable.
• Communication Skills: Strong communication skills, both written and verbal, are essential for this position. You must demonstrate effective interpersonal skills to engage with diverse audiences, including participants, team members, and community partners.
• Technical Proficiency: A basic level of computer skills is required, including familiarity with Microsoft Word and other relevant software. Candidates should be comfortable navigating digital platforms to complete necessary documentation and communication.
• Bilingual Skills: Proficiency in both English and Spanish is highly preferred, as it allows for better engagement with a diverse population and enhances our outreach efforts.
• Physical Requirements: Candidates must be physically capable of lifting boxes and supply packs weighing up to 20 pounds. You should also be prepared for a role that may involve physical activity, walking and mobility.
• Bilingual (English and Spanish) preferred
