Responsibilities:
- Provide direct patient care in home settings in accordance with physician-directed care plans
- Monitor and document patient health status, vital signs, and progress notes
- Administer medications, wound care, and other skilled nursing treatments
- Communicate effectively with patients, families, and the care team regarding patient conditions and changes
- Educate patients and caregivers on health management, medications, and safety
- Maintain accurate and timely clinical documentation in compliance with state and agency standards
- Collaborate with RNs, therapists, and other healthcare professionals to ensure continuity of care
- Active and unrestricted LPN license in the state of Florida
- Previous home health or clinical nursing experience preferred
- Strong knowledge of skilled nursing procedures, medication administration, and wound care
- Excellent communication, documentation, and time management skills
- Reliable transportation and valid driver's license required
- CPR/BLS certification required or ability to obtain upon hire
- Compassionate, patient-centered approach with a strong sense of professionalism
