We are seeking a dedicated individual who will be responsible for guiding seniors through the complex process of applying for Medicaid, providing support with document collection, form filling, and making necessary phone calls.
Key Responsibilities:
- Manage a caseload of seniors who qualify for Medicaid
- Assess seniors' needs and provide necessary assistance
- Conduct home visits and collect relevant documents
- Follow up to ensure timely receipt of documentation
- Drive to patient homes within Cuyahoga County and surrounding areas
- Coordinate with Medicaid offices to submit required forms
- Close cases upon successful approval of Medicaid benefits
This is a short-term position and offers a schedule from 9 a.m. to 5 p.m., Monday through Friday, allowing you to make a meaningful impact on the lives of seniors in our community. A valid driver's license and access to a car are required for this role.

Comfort working with seniors and their families is a plus, as well as holding a valid Driver's License and necessary insurance coverage. Candidates must be able to pass a background and BMV check to be considered for this position.
