We are seeking Case Managers to assist older adults needing long-term care services to remain in the community. Must have good assessment skills and be a Registered Nurse (RN), Licensed Practical Nurse (LPN), Licensed Social Worker (LSW) or Licensed Social Worker Trainee (SWT) holding an active license in the State of Ohio.
