What are the responsibilities and job description for the RN/Caseworker position at Area Agency on Aging of West Central Arkansas?
JOB SUMMARY:
The RN/Caseworker works under the supervision of the In-Home Regional Manager/RN and Director of Client Services. Monitors clients to assure that services provided will be individual to each client and provided in the least restrictive environment. Develops the service plan assuring participation of those concerned with the client’s welfare; monitors and adjusts the service plan to reflect changes in the client’s needs; provides the system that safeguards the client’s rights. Complies with all laws and regulations pertaining to the program. Provides technical assistance to the In-Home coordinator, secretary Casemanager and HCAs as needed. Performs all client assessments, reassessments and visits when due or more often if needed. Assists Home Care Supervisor with Aide evaluations. Performs targeted Case Management on assigned clients.
DUTIES & RESPONSIBILITIES:
1. Assess new referrals within 72 hours of receiving from Case Manager (at least make contact/appointment).
2. Perform initial evaluation/assessment of new clients prior to receiving services determining current needs, physical disabilities, record vital signs and incorporate into the service plan as well as complete DMS 618’s and all other forms required.
3. Determine during assessment or reassessment with input from client/family member how often Monitoring visit is needed and DOCUMENT in Wellsky.
4. Perform yearly reassessments for needed changes in service plans and to assure services are being provided as ordered yearly reassessments will be done when Aides are not present.
5. Perform initial visits for VA clients, complete service plan.
6. Schedule in Wellsky Perform 62-70 day monitoring visits and complete billing for all VA clients.
7. Upload required forms into client file in Wellsky.
8. Perform TCM as assigned or needed for PP and VA clients.
9. Insure all client records are complete in Wellsky. All care plans are COMPLETE in Wellsky in a timely manner so Caregivers have a plan to work from.
10. Assist and cooperate fully with all officials conducting required audits and surveys.
11. Assist in planning and conduction bi annual In-service for caregivers per Medicaid guidelines.
12. Follow all Medicaid rules and regulations and monitor at all times for changes.
13. Assist Home Care Supervisor in counseling and provide correction to Aides performance when needed.
14. Investigate client complaints and provide needed resolution along with team. Report issues/ complaint summary to DCS.
15. Answer all medical related calls from Caregivers.
16. Participate in Agency Marketing Activities / Health Fairs as assigned.
17. Develop solutions in conjunction with other staff to health care needs of the area and maintain public relations with other health are professionals and the general public.
18. Attend and participate in all weekly IN HOME/ Case Management meetings.
19. Cross train with Aide Coordinator and Secretary.
20. Assess/ certify PCA/CNA skills check off upon hire when necessary.
21. Complete service plans and contracts for PP clients, Personal Care and Homemakers.
22. Coordinate services with other agencies as needed.
23. Adhere to Code of Ethics and Agency Policy and Procedures as well as State Board of Nursing regulations.
24. Cross train with Case Managers.
25. Cross train to provide PCA classes when needed or requested.
26. Handle Worker’s Compensation claims to the point od sending to Human Resources.
27. Assist Aide Coordinator, Secretary or Home Care Supervisor as needed/requested.
28. Because the job is considered a safety-sensitive position, the employee in this position will be subject to unannounced drug and/or alcohol testing as permitted by law.
29. The employee must be able to lift up to 50 pounds on a regular basis.

The formal education equivalent of an Associate Degree, Diploma, or Bachelor Degree in Nursing, at least two years experience in nursing.
Registered by the Arkansas State Board of Nursing
