POSITION OVERVIEW:
An Aging Care Manager 3 position for the Area Agency on Aging of Somerset County in the Protective Services Program and is responsible for maintaining a caseload of vulnerable clients who are in need of protection from abuse, exploitation, abandonment or self-neglect. This individual may be required to provide specialized casework services to clients who are having difficulty maintaining a safe and healthful independent lifestyle, and, therefore this individual is expected to be knowledgeable with the protective services legislation, rules, regulations, and operating procedures involved in the protective services program. An employee in this class coordinates medical, social, psychological, or functional evaluations of the client’s condition and must adhere to established legal and statutory time frames which provide little or no flexibility. An Aging Care Manager 3 is expected to have sufficient knowledge of community resources to be able to include them as a factor in assessing the client’s circumstances and to participate in formal and informal training sessions, which provide the methods, procedures, rules, and regulations that govern the program and the internal operations of the Area Agency on Aging to which the employee is assigned. This individual is also provided with an increasingly more difficult series of work assignments during their employment in this class title. As knowledge and skills are acquired, the difficulty of assignments increase through individual and group conferences, reading and evaluating records and reports, assignments of professional reading, attendance at training classes, and other training devices. This individual will complete psychosocial assessments and develop care plans that will define met and unmet needs of the consumer. This work requires that the employee document all actions and pay close attention to detail without receiving close supervision. Work is performed in accordance with established regulations, policies, and procedures and is reviewed by a supervisor through conferences, reports, and the resolution of difficult situations. Monitoring will be required to assure that care needs are being met according to the standards defined by the Department of Aging. This individual will be required to complete data entry within the statewide reporting systems known as Statewide Data Reporting System for each consumer assigned to their caseload. An Aging Care Manager 3 is required to complete assessments, assist with on-call rotation within the Protective Services Program, and perform other related duties within the defined Aging program, as needed.
EDUCATION/EXPERIENCE:
Any combination equivalent to education and experience that provides the required skill and knowledge is qualifying. Typical qualifications would be equivalent to:
One year as an Aging Care Manager 2 or Aging Program Assessor; or
three years of experience in public or private social work, including one year of direct care management experience and a Bachelor's Degree which includes or is supplemented by 12 semester hours credit in sociology, social welfare, psychology, gerontology, or other related social sciences; or
any equivalent combination of experience and training including successful completion of 12 semester hours of college level courses in sociology, social welfare, psychology, gerontology, or other related social sciences.
EMPLOYMENT STANDARDS:
An employee in this class title must possess knowledge of the laws, rules, and regulations governing the protective services program, knowledge of client conditions that permit or require the intervention of court supervision, and knowledge of social casework principles and methods. This individual must also possess knowledge of the availability of community support services for the benefit of elderly clients, knowledge of current social, economic and health problems and resources as they relate to older adults, and knowledge of individual and group behavior and ways of working effectively with older adults who have social, economic, emotional, or health problems. An employee in this class title must possess the ability to plan and organize work, prepare adequate records and reports, set priorities and maintain a consumer caseload in an effective and timely manner, the ability to clearly express ideas orally and in writing and to interpret laws and regulations, and the ability to establish and maintain effective working relationships with consumers, other members of the staff, outside agencies and institutions and the general public. This individual must also possess skill in the development of plans to address the needs of vulnerable older adults, the ability to maintain confidentiality in high pressure situations, the ability to understand and accept the needs and rights of others and work with adults who are physically, mentally, emotionally disabled or economically disadvantaged, and the ability to work effectively with people and aid them to grow in the constructive use of their potential in adjusting to their specific problems. Computer skills are required to access, manage, and monitor consumer records through the Statewide Data Reporting System.

Responsibilities
An employee in this classification is responsible for maintaining a caseload of vulnerable clients who are in need of protection or mental health case management services that are part of the overall program of the Agency. Assignments are carried out in accordance with established federal, state, and local regulations and policies. Specific job duties include the following:
- Conducts thorough investigations of the circumstances of clients who need protection from abuse, exploitation, abandonment, or self-neglect.
- Recommends cases for court intervention, including guardianships or mental health commitments.
- Adheres to all documentation requirements as outlined in the Protective Services regulations and Statewide Data Reporting System.
- Establishes an effective working relationship with clients, family members, pastors, neighbors, other social service workers, or attorneys in order to provide a better living situation for an assigned client.
- Obtains information from clients, members of clients’ families, neighbors, and other persons necessary for the identification of social, economic, emotional, health, or physical problems, and for assisting clients in obtaining a variety of services for older adults on the basis of proper procedures and established policies.
- Prepares social assessment forms and establishes a care plan which includes obtainable goals to address client needs.
- Testifies in court concerning various aspects of assigned cases.
- Arranges for representative payee to protect the clients’ financial assets.
- Perform other related duties in the defined aging program as needed.
- Attends work on a regular basis.
- Maintains a professional appearance and demeanor.
- Attends all required meetings and trainings.
- Adheres to and promotes maintenance of Agency confidentiality guidelines.
- Adheres to and promotes Agency policies and procedures.
- Operates and maintains Agency vehicles in a safe manner.
