The Area Agency on Aging of Broward County (AAABC) is a nonprofit organization dedicated to serving adult residents, age 60 or older throughout Broward County. As the leading advocate for older adults in our community, AAABC plans, develops, coordinates, and evaluates programs; funds essential services, and provides a central source of information for older adults, their families, and caregivers.
Position Summary
The Healthcare Navigator serves as the primary point of contact for caregivers seeking assistance. This role helps families understand available services, conducts standardized assessments, provides education on caregiver support programs, and ensures caregivers feel supported throughout their journey. The ideal candidate is empathetic, organized, and comfortable working directly with the public.
Key Responsibilities
- Conduct caregiver intake screenings, assessments, and follow-up calls.
- Complete formal caregiver assessments to identify stress levels, support needs, and tailored service recommendations.
- Offer individualized guidance on available caregiver support programs, respite services, and community resources.
- Maintain accurate documentation and case notes in the client database.
- Participate in outreach activities, including mobile office hours, community presentations, local events, and partnerships with municipal and nonprofit agencies.
- Collaborate with internal departments and external partners to ensure coordinated care.
- Monitor caregiver progress and provide ongoing support as needs evolve.
- Ensure compliance with program guidelines, confidentiality standards, and agency policies.
Qualifications: A Bachelor's Degree (B.A/B.S.) from an accredited College or University in Social Work is preferred. Social work experience preferred. Proficiency in Microsoft Word, PowerPoint, Gmail and Google Sheets.

Pay: $23.74 - $28.59 per hour
