Physical Therapist (PT) — Home Health

Duration: 13 Weeks (option to extend)

CenterWell Home Health is seeking a dedicated Physical Therapist for a travel contract in Rolla, MO. This role offers the opportunity to provide one-on-one care in patients’ homes, focusing on improving mobility, independence, and overall quality of life.
Key Responsibilities- Evaluate and treat patients in a home health setting
- Develop and implement individualized care plans
- Monitor patient progress and adjust treatment as needed
- Educate patients and caregivers on exercises and safety
- Complete timely documentation (OASIS experience preferred)
- Collaborate with interdisciplinary healthcare teams
- Active Missouri PT license (or ability to obtain)
- Home Health experience preferred (HCHB & OASIS a plus)
- Strong clinical and communication skills
- Ability to work independently and manage a caseload
- BLS certification
- Weekly Pay: $2,650 (blended package)
