• Provide skilled physical therapy services to residents in a SNF setting.
• Perform evaluations and develop individualized treatment plans.
• Help patients improve strength, balance, mobility, and functional independence.
• Assist patients recovering from illness, injury, surgery, or hospitalization.
• Collaborate with nursing staff, physicians, and the interdisciplinary rehabilitation team.
• Complete accurate and timely clinical documentation.
