Social Worker - Home Health

We are seeking a compassionate and dedicated Licensed Social Worker to join our team. The ideal candidate will have a strong background in geriatrics and home health care, with experience supporting patients in an in-home or community-based setting.

Role

In this role, you will provide a comprehensive range of social work services to home health patients and their families, including:

• Conducting initial psychosocial, emotional, and environmental assessments
• Providing ongoing counseling, education, and support to patients and caregivers
• Assisting with community resources, referrals, and discharge planning
• Collaborating with the clinical team to promote patient independence and well-being
  
  

As an essential member of the Interdisciplinary Team (IDT), the Home Health Social Worker plays a vital role in assessing patient needs, developing individualized care plans, and ensuring services align with home health standards and regulatory requirements.
