Position Overview
The Director of Nursing (DON) is responsible for clinical oversight, regulatory compliance, resident outcomes, and overall quality of care across the community. This role provides leadership over all clinical functions, ensures adherence to DHS 83 requirements, and supports the development and performance of clinical staff. The DON works closely with the Executive Director and leadership team to maintain high standards of care and operational excellence.

This position includes on-call for clinical support and limited weekend coverage as needed.

Key Responsibilities

Clinical Leadership & Oversight

  • Maintain full accountability for clinical outcomes, compliance, and quality of care
  • Oversee medication administration practices at a supervisory level
  • Provide nurse delegation and ensure safe care delivery by staff
  • Ensure adherence to standard precautions and infection control protocols

Staffing, Training & Clinical Supervision

  • Conduct interviews and participate in hiring of care staff
  • Oversee initial clinical training and onboarding for new hires
  • Provide dementia-specific training for all staff
  • Oversee required CBRF training programs, including:
  • Standard Precautions
  • First Aid and Choking Response
  • Fire Safety
  • Medication Administration
  • Approve and monitor ongoing continuing education (in coordination with Staffing Coordinator)
  • Conduct annual clinical performance reviews and provide input on compensation (East & West)
  • Address clinical performance issues, including disciplinary actions when necessary

Resident Care & Regulatory Compliance

  • Ensure compliance with DHS 83 and submit required reporting and waiver requests
  • Maintain oversight of all resident medical records and documentation
  • Oversee psychotropic medication reviews and clinical assessments
  • Develop and approve Individualized Service Plans (ISPs)
  • Evaluate and approve residents for admission (clinical approval)
  • Perform and oversee evacuation assessments
  • Conduct annual and change-of-condition assessments
  • Coordinate and participate in care conferences with families
  • Communicate proactively with families regarding clinical concerns
  • Coordinate care with physicians, pharmacies, and other healthcare providers
  • Implement and oversee fall reduction initiatives

Quality Assurance & Leadership

  • Assist Executive Director with Quality Assurance (QA/QAPI) preparation, goals, and outcomes
  • Plan and lead monthly clinical meetings with Executive Director and Staffing Coordinator
  • Participate in monthly QAPI meetings
  • Monitor and investigate incidents (including falls) and ensure interventions are implemented

On-Call & Coverage Expectations

  • Serve as the primary on-call contact for clinical support needs
  • Provide weekend Manager on duty coverage as needed
  • Share staffing on-call responsibilities on designated weekends with ADON and Staffing Coordinator

Qualifications

  • Active Registered Nurse (RN) license in the state of Wisconsin
  • Experience in assisted living, memory care, or long-term care preferred
  • Strong knowledge of DHS 83 regulations
  • Proven leadership experience with staff supervision and training
  • Strong organizational, communication, and problem-solving skills
