JOB TITLE: Public Health Practitioner (MD or NP) - Women's Health & Primary Care

DEPARTMENT: Public Health

FLSA STATUS: Non-Exempt, Part-Time (10-15 hours/week)


Schedule: Flexible, to be determined in collaboration with clinic leadership.

About Us: Archuleta County Public Health Department has a compassionate and patient-centered clinic. We are starting new programming to provide comprehensive Women's Health and other primary care services to the Pagosa Springs community. We believe in a holistic approach to health, emphasizing preventative care, education, and community well-being.


Position Summary:

The Public Health Practitioner (MD or NP) will play a vital role in providing direct clinical services as well as integrating public health principles into our clinical practice, focusing on health promotion, disease prevention, and community-level interventions for our women's health and primary care patient population. This role is ideal for a physician or nurse practitioner passionate about population health, health equity, and empowering individuals to achieve optimal health outcomes.


Key Responsibilities:

  • Direct Patient Care
    • Provide women's reproductive health support including annual exams.
    • Provide annual exams and preventative health care appointments and screenings.
    • Provide evidence-based counseling on healthy lifestyles, nutrition, physical activity, and stress management.
    • Empower patients with knowledge and resources to make informed health decisions.
    • Support development of new clinical services as ACPHD continues to expand
  • Data Analysis & Quality Improvement (QI):
    • Contribute to understanding and addressing unique public health challenges within the local population.
    • Assist in collecting and analyzing health data to identify population health trends, disparities, and areas for intervention within the clinic's patient base.
    • Participate in quality improvement initiatives aimed at enhancing patient outcomes and public health impact.
    • Monitor and report on key public health metrics.
    • Participate in key opportunities that are required by funders for elements of the program.

Qualifications:

  • Education:
    • Medical Doctor (MD/DO) degree from an accredited institution AND completion of a residency program in Family Medicine, Internal Medicine, Obstetrics and Gynecology, or Preventive Medicine.
    • OR
    • Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited institution with Nurse Practitioner (NP) certification in Family Health, Women's Health, or Adult-Gerontology Primary Care.
  • Licensure/Certification:
    • Current, unrestricted Medical License in the State of Colorado (for MD/DO).
    • Current, unrestricted Registered Nurse (RN) license and Advanced Practice Registered Nurse (APRN) license in the State of Colorado (for NP).
    • Board Certification in your respective specialty (if applicable).
    • Current Basic Life Support (BLS) certification.
  • Experience:
    • 2-3 years of clinical experience in primary care or women's health preferred
    • Demonstrated experience or strong interest in public health, population health, health education, or community health initiatives.
    • Experience working with diverse patient populations.
  • Skills & Abilities:
    • Bi-Lingual preferred
    • Excellent communication and interpersonal skills, with the ability to engage effectively with patients, staff, and community partners.
    • Strong analytical and problem-solving abilities, particularly related to health data and trends.
    • Ability to work independently and collaboratively within a team environment.
    • Proficiency in electronic health records (EHR) systems.
    • Knowledge of local public health resources and community needs in Pagosa Springs, Colorado, is a plus.
    • Commitment to health equity and patient-centered care.

  • Working Conditions: Work is performed in both in homes of community families and in an indoor office environment with moderate exposure to atmospheric conditions (such as fumes, odors, dusts, gases, poor ventilation, etc.); moderate exposure to hazardous materials (such as chemicals, radioactive materials, zoonotic or communicable diseases, blood and other body fluids); and moderate exposure to extreme temperatures and travel.
  • Occasional evening and/or weekend work may be required depending on the assignment.
  • Emergency Preparedness: Must participate from time to time in Disaster Preparedness and emergency training and events.
