What are the responsibilities and job description for the DISCHARGE PLANNER position at Archbold Medical Center?
Discharge Planner- Northside
Qualifications:
Minimum Bachelors degree
Minimum of one year's relevant inpatient experience
Ability to effectively assess, plan and implement discharge planning services in a multidisciplinary setting. Particular skills in community resources and/or the ability to research appropriate community resources and to mobilize and coordinate resources effectively for patient aftercare needs. Knowledgeable about needs of dually diagnosed, medically compromised, and elderly populations. Knowledge of utilization review and management.
Good physical and mental health. Able to work under supervision or independently as directed by the situation. Must be able to work under stressful conditions.
Must be able to concentrate on work amid distractions such as noise, conversations, and foot traffic; must be able to consistently meet deadlines regardless of caseload; must be flexible in work hours in order to meet patient and Inpatient Hospital Program operating needs.
Must maintain self-control in volatile or hostile situations such as when verbally or physically confronted.
Must be able to set and maintain therapeutic distance with clients; maintain therapeutic boundaries during treatment and following discharge.
Must be able to address and prioritize multiple task demands within established time frames. Proficiency with computers is necessary.
Responsibilities:
Develops new sources for referrals when completing discharge plans and keeps staff informed of new sources.
Identifies discharge plans appropriate to age and situation for all clients.
Participates in treatment planning with physicians and communicates changes in discharge plans as necessary
Transfers all information to referral sources necessary to complete discharge plans.
Arranges and conducts family meetings when appropriate to address discharge plans.
Participates in discharge planning in collaboration with the patient/family, physician, and healthcare team that meets mutually set goals, is clinically desirable, and is financially feasible.
In collaboration with nursing, coordinates discharge teaching and ensures discharge orders/instructions and prescriptions are written prior to patient discharge.
