Job Title: Diabetes Educator
Job Type: Full-time (Monday-Friday, 8:00 AM - 5:00 PM)
Travel: 0-15%
Qualifications:
? Associate’s degree in a related field required; Bachelor’s degree preferred: Certified Diabetes Care and Education Specialist is preferred.
? Current RN license in the appropriate state required
? Minimum of 1 year of experience in a medical facility, with prior case management experience preferred
? Proficiency in Microsoft Word, Excel, and electronic record-keeping systems
? CPR and TB certification at the time of hire, renewed annually
Job Summary:
Discover a rewarding career at Arcare. We are seeking a skilled, compassionate, and detail-oriented Diabetes Educator to join the Arcare team. In this role, you will be to plan and coordinate the efforts of the multidisciplinary team providing diabetes management and education to patients in person and via Telehealth
The ideal candidate will demonstrate strong communication and organizational skills, the ability to navigate complex healthcare needs, and a passion for empowering patients to achieve better health outcomes. Key Responsibilities:
? Identify the needs of each patient and build a care plan based on those needs.
? Coordinate and facilitate the activities of the diabetic patients and/or their significant others during the diabetic education classes.
? Coordinate with healthcare providers and community resources to support patient care.
? Conduct patient monitoring through phone, telehealth, secure messaging, or in-person interactions.
? Document patient interactions, progress notes, and care plans in the CAREWare database and other systems.
? Participate in case conferences, outreach events, and training sessions to enhance care delivery.
? Educate patients and families about self-management goals, healthy behaviors, and available support services.
? Collaborate with federal, state, and local agencies to maximize resources and achieve program development goals.
? Support team members with patient care tasks, quality improvement initiatives, and special projects as needed.
