Job Title: Chronic Disease Manager RN

Job Type: Full-time (Monday-Friday, 8:00 AM - 5:00 PM)

Travel: 0-15%

Qualifications:

• Associate's degree in a related field required; Bachelor's degree preferred
• Current LPN or RN license in the appropriate state required
• Minimum of 1 year of experience in a medical facility, with prior case management experience preferred
• Proficiency in Microsoft Word, Excel, and electronic record-keeping systems
• CPR and TB certification at the time of hire, renewed annually
• Valid driver's license and reliable transportation

Discover a rewarding career at Arcare. We are seeking a skilled, compassionate, and detail-oriented Chronic Disease Case Manager LPN or RN to join the Arcare team. In this role, you will provide critical education and case management support to primary care patients and clients living with chronic diseases, including those with HIV. By collaborating with medical providers, care teams, and community resources, you will ensure patients receive personalized, comprehensive care to manage their conditions effectively.

Key Responsibilities:

• Develop individualized Medical Services Plans for patients within 30 days of enrollment, updating them based on acuity and care standards.
• Provide ongoing case management and education on chronic disease management, including medication adherence, health maintenance, and disease prevention.
• Coordinate with healthcare providers and community resources to support patient care.
• Conduct patient monitoring through phone, telehealth, secure messaging, or in-person interactions.
• Document patient interactions, progress notes, and care plans in the CAREWare database and other systems.
• Participate in case conferences, outreach events, and training sessions to enhance care delivery.
• Educate patients and families about self-management goals, healthy behaviors, and available support services.
• Collaborate with federal, state, and local agencies to maximize resources and achieve program development goals.
• Support team members with patient care tasks, quality improvement initiatives, and special projects as needed.
