The Licensed Social Worker provides psychosocial support to residents and families in a skilled nursing and assisted living environment. This role ensures compliance with state and federal regulations while promoting dignity, independence, and quality of life for residents.

Key Responsibilities
- Conduct initial and ongoing psychosocial assessments for residents.
- Develop and implement individualized care plans in collaboration with interdisciplinary teams.
- Provide counseling and crisis intervention for residents and families.
- Facilitate discharge planning and coordinate community resources.
- Document social service needs and progress per regulatory requirements.
- Advocate for residents’ rights and assist with conflict resolution.
- Participate in care plan meetings and quality assurance initiatives.
- Current Ohio LSW license (required).
- Bachelor’s degree in Social Work; Master’s preferred.
- Experience in long-term care or healthcare setting strongly preferred.
- Knowledge of Medicare/Medicaid regulations and discharge planning.
- Strong communication, organizational, and problem-solving skills.
Salary : $55,000 - $67,000
