POSITION OVERVIEW:The Case Manager provides medical social services support to the client and/or caregiver according to the Plan of Care.
ESSENTIAL JOB FUNCTIONS:
Must be able to read, write, speak and understand the English language.
Must be able to sit, stand, bend, lift, push, pull, stoop, walk, and move intermittently throughout the work day.
Must practice regular and dependable attendanceUsing effective body mechanics must be able to lift 15 lbs. floor to waist; lift 15 lbs. waist to shoulder; lift and carry 15 lbs.; and push/pull 15 lbs.
Must be able to sit in a normal seated position for extended periods of time.
Must be able to concentrate and use reasoning skills and good judgment.
SPECIFIC REQUIREMENTS:
Must have excellent verbal and written communication skills, organizational skills, great attention to detail, and analyticalMust understand and follow company policies and procedures, including harassment and complaint procedures.
Must possess the ability to make independent decisions when circumstances warrant such actions.

Must have a pre-employment physical examination by a licensed physician in Massachusetts prior to employment, showing examination within 12 months before hire date, to be in good health including freedom from communicable disease, including tuberculosis in communicable form, according to acceptable screen methods of the DPH.Must be able to pass a criminal background check and a satisfactory reference check.
EDUCATION AND EXPERIENCE:
Baccalaureate degree in an appropriate course of study from an institution accredited institution, has 1 year of experience in a healthcare setting and is supervised by a MSW.
WORKING CONDITIONS:
Works in fast paced center environment under a medical modelSubject to frequent interruptions.
Involved with client, family members, associates, visitors, government agencies/personnel, etc., under all conditions and circumstances.
Possible exposure to infectious waste, diseases, conditions, etc., including exposure to the AIDS and hepatitis B viruses.
Reports to the Program Director.
GENERAL RESPONSIBILITIES:
Conducts a psychosocial assessment as part of the comprehensive assessment upon a client’s admission to the program. This includes emotional, social, financial and environmental resources and to identify appropriate psychosocial problems, interventions and goals for the client’s plan of care.
Participate in the development of the client’s plan of care.
Observe, record and report changes on the client’s emotional and social factors that are affecting the client’s quality of life.
Maintain and submit written clinical records as deemed by the Agency, including the initial evaluation the care plan and daily notes.
Lead the discharge planning process and follow-up.Help participants to utilize the resources of their community.
