What you will do:

  • Develop, implement, and oversee Individual Support Plans, outcomes, and Individual Family Service Plans for PLT participants
  • Provide home and community-based services to an assigned caseload of four participants, supporting parents with decision-making and crisis and non-crisis situations using a person-centered approach
  • Develop and implement Parent Café and assist in facilitation of group sessions
  • Coordinate and track medical and dental appointments for children of PLT participants and attend meetings related to school-aged children on assigned caseload=
  • Update Adult Program curriculum as needed 
  • Manage and maintain data in varied databases/systems
  • Create and maintain reporting functions that allow the ability to analyze information 
  • Review staff documentation in Salesforce database for accuracy
  • Support and train staff in databases/systems, including daily documentation and billing requirements 
  • In conjunction with Human Resources and the Program Coordinator, train, onboard, and manage PLT staff, as well as supervise volunteers and university students.
  • Develop and maintain relationships with service providers and funding sources and serve as a liaison regarding program requirements, deadlines, and participant services.
  • Assist Coordinator in planning program community outings, annual program events
  • Assist with program purchasing and other operational needs, including coordinating maintenance of the program van.

  • Bachelors Degree in Human Services, Special Education, or related field
  • Three or more years of related experience 
  • At least one year of supervisory experience
