Centralized Plan Coordinator

As a Centralized Plan Coordinator, you'll play an essential role in developing and coordinating person-centered service plans that support each individual's goals, strengths, and aspirations.
About the Role
The Centralized Plan Coordinator partners with individuals receiving services, their families, and interdisciplinary teams to create, implement, monitor, and update individualized service plans. This position ensures services are delivered in accordance with person-centered planning principles, agency standards, and regulatory requirements while promoting quality outcomes for the people we support.
What You'll Do
- Develop, coordinate, and maintain person-centered service plans.
- Collaborate with individuals, families, care providers, and interdisciplinary teams to identify goals and support needs.
- Monitor implementation of individualized plans and ensure services align with established goals.
- Maintain accurate, timely, and compliant documentation.
- Coordinate plan reviews and updates as individuals' needs and goals evolve.
- Ensure services meet agency policies as well as state and federal regulations.
- Advocate for individuals to ensure their voices, choices, and preferences remain central to the planning process.
- Build positive relationships with internal teams and community partners to support successful outcomes.
What We're Looking For
Required Qualifications
- Bachelor's degree in Human Services or a related field.
- At least one year of experience supporting individuals with developmental disabilities.
- Valid New York State driver's license with a driving record that meets agency insurance requirements.
Preferred Skills
- Strong organizational and time-management abilities.
- Excellent written and verbal communication skills.
- Experience coordinating services and maintaining detailed documentation.
- Ability to work collaboratively with individuals, families, and multidisciplinary teams.
