The LPN/LVN delivers care under the direction of a Registered Nurse (RN) and in compliance with applicable state licensure, regulatory standards, and organizational protocols.

The LPN/LVN delivers care under the direction of a Registered Nurse (RN) and in compliance with applicable state licensure, regulatory standards, and organizational protocols.
Key Responsibilities
- Provide skilled nursing care to patients and families under the supervision of a Registered Nurse and in alignment with the plan of care.
- Support symptom management and comfort-focused care during periods of crisis, providing direct bedside care in the patient's residence setting.
- Monitor and document changes in condition, escalating to the RN Case Manager and Director/Manager of Clinical Services as appropriate.
- Educate caregivers on personal care, medication administration, and end-of-life expectations to ensure safe and confident care between visits.
- Provide emotional support to patients and families and participate in critical moments, including time-of-death care and after-death procedures.
- Collaborate with the interdisciplinary group (IDG) and external care partners to ensure coordinated, patient-centered service delivery.
- Complete all clinical documentation in the electronic medical record timely, accurately, and in compliance with agency policy and regulations.
- Attend team meetings, in-services, and compliance trainings, and support additional responsibilities within licensure and scope as needed.
- Performs other duties as required.
- Current LPN or LVN license in good standing in the state of practice or a valid compact license covering all states of operation.
- Minimum of one (1) year of clinical nursing experience preferred, ideally in hospice, home health, or geriatrics.
- Demonstrated understanding of hospice philosophy, symptom management, and end-of-life care practices.
- Proficiency in electronic medical record (EMR) documentation and mobile clinical tools.
- CPR certification required.
- Valid driver's license, reliable transportation, and proof of automobile insurance.
- Strong communication skills and the ability to function effectively as part of an interdisciplinary care team.
