This is a full time position supporting hospice patients across Marion County, Monday to Friday with a weekend rotation. Assignments may vary based on patient needs and coverage requirements.
Overview
The Licensed Practical Nurse / Licensed Vocational Nurse (LPN/LVN) provides direct nursing care to patients in their homes or care settings under the supervision of a Registered Nurse. This role supports the delivery of hospice care in accordance with the plan of care, agency protocols, and the nurse's scope of practice. The LPN/LVN performs clinical visits including routine nursing care, symptom management, and support during acute changes in condition. This team member may also be scheduled for Continuous Care assignments or assist with after-hours needs as part of the on-call rotation.
Key Responsibilities
- Provide direct nursing care to hospice patients under the supervision of a Registered Nurse, in alignment with the plan of care and scope of practice.
- Perform scheduled and unscheduled nursing visits, including comfort care, medication administration, and skilled observation.
- Document all care and communication accurately and promptly in the electronic medical record (EMR).
- Support pain and symptom management interventions, reporting changes in condition to the supervising RN or physician as appropriate.
- Collaborate with interdisciplinary team members to ensure coordinated, patient- and family-centered care.
- Educate patients and families on care techniques, medications, and symptom awareness as directed.
- Provide support for emergency visits, tuck-in calls, respite, and Continuous Care as appropriate.
- Participate in on-call rotations and respond to after-hours clinical needs when scheduled.
- Maintain compliance with agency policies, hospice regulations, and professional standards of practice.
- Performs other duties as required.
Qualifications
- Current LPN or LVN license in good standing in the state of practice or a valid compact license covering all states of operation.
- Minimum of one (1) year of clinical nursing experience preferred, ideally in hospice, home health, or geriatrics.
- Demonstrated understanding of hospice philosophy, symptom management, and end-of-life care practices.
- Proficiency in electronic medical record (EMR) documentation and mobile clinical tools.
- CPR certification required.
- Valid driver's license, reliable transportation, and proof of automobile insurance.
- Strong communication skills and the ability to function effectively as part of an interdisciplinary care team.
