Role Summary
In this role, you will step in when patients and families need skilled nursing presence the most. As a PRN Hospice LPN – Continuous Care, you will provide calm, steady nursing care during critical moments, offering comfort and reassurance through extended bedside support. This role is ideal for a nurse who values focused, one-to-one care and being fully present during hospice crisis situations.
Overview
The Hospice LPN/LVN – Continuous Care provides direct, high-acuity nursing care during episodes of Continuous Care (CHC) in accordance with the patient's individualized plan of care. This role supports patients experiencing a hospice-appropriate crisis requiring extended skilled nursing services in the home or facility setting. Care is delivered under the direction of a Registered Nurse and in compliance with applicable licensure, regulatory standards, and organizational protocols.
Key Responsibilities
- Provide skilled nursing care to patients and families under the supervision of a Registered Nurse and in alignment with the plan of care
- Support symptom management and comfort-focused care during periods of crisis, providing continuous bedside nursing support
- Monitor and document changes in patient condition, escalating concerns to the RN Case Manager and clinical leadership as appropriate
- Educate caregivers on personal care, medication administration, and end-of-life expectations to support safe care between visits
- Provide emotional support to patients and families, including participation in time-of-death care and after-death procedures
- Collaborate with the interdisciplinary group (IDG) and external care partners to ensure coordinated, patient-centered care delivery
- Complete clinical documentation accurately and timely in the electronic medical record in accordance with policy and regulatory requirements
- Participate in team meetings, in-services, and required compliance trainings
- Perform other duties within licensure and scope as assigned
Qualifications
- Current LPN or LVN license in good standing in the state of practice or a valid compact license covering applicable states
- Minimum of one (1) year of clinical nursing experience preferred, ideally in hospice, home health, or geriatrics
- Demonstrated understanding of hospice philosophy, symptom management, and end-of-life care practices
- Proficiency with electronic medical record documentation and mobile clinical tools
- CPR certification required
- Valid driver's license, reliable transportation, and proof of automobile insurance
- Strong communication skills and ability to function effectively as part of an interdisciplinary care team
