WHAT WOULD I BE DOING?
· Working with individuals with intellectual and/or developmental disabilities to develop and maintain a circle of support and planning team that best supports their best interests
· Assisting individuals develop a Self-Direction budget consistent with their plan and services
· Attend semi-annual and annual service plan reviews
· Provide education, training, and technical assistance to individuals and their Self-Hire staff
· Maintain Support Broker authorization on an annual basis
- Maintain program related records and documentation
- Provide support broker services based on OPWDD guidelines
QUALIFICATIONS:
- Bachelor’s and/or Associate’s Degree in Human Services or related field with one-year experience working with Individuals with Developmental Disabilities. High School Diploma with comparable education and/or experience may be considered.
- Valid NYS driver’s license with a safe driving record per agency policy.

Pay: $22.00 - $24.00 per hour
