Key Responsibilities
- Lead, coach, and develop care coordination teams; oversee hiring and performance management
- Oversee care planning, discharge processes, and complex case escalations
- Ensure coordination across clinical teams, families, and community partners
- Monitor workflows, KPIs, and departmental performance (access, throughput, outcomes)
- Drive process improvement, documentation integrity, and reimbursement optimization
- Ensure compliance with HIPAA, CMS, and accreditation standards
- Partner with leadership, finance, and patient access on strategic initiatives
- Build and maintain external partnerships to support care transitions

Requirements
Required:
- Bachelor’s degree in healthcare administration, business, social work, nursing, or related field
- 2 years of case management experience
- Behavioral health or inpatient experience
- 1–2 years of leadership experience
- 5 years in case management, patient access, or healthcare financial services
CPR, CPI, CASII, and Ukeru required within 3 months of hire
