At Springfield Masonic Community, our therapy department plays a vital role in supporting the health, independence, and quality of life for residents across our entire CCRC campus. We provide comprehensive therapy services in multiple settings to meet a wide range of needs:
- Outpatient Therapy: Helping active residents maintain and restore mobility, strength, and overall wellness through individualized treatment plans.
- Pathways Dementia Care: Providing compassionate, evidence-based therapy interventions that focus on functional ability, safety, and quality of life for those living with memory care needs.
- Assisted Living: Supporting residents in maintaining independence with targeted therapies that address mobility, balance, and daily living activities.
- Skilled Nursing and Long-Term Care: Delivering intensive rehabilitation following illness, surgery, or injury, as well as ongoing therapy to enhance comfort, prevent decline, and promote dignity.
Occupational Therapy Assistant

We are currently seeking a committed and patient-focused Occupational Therapist Assistant to provide rehabilitation services in one of our locations. The Occupational Therapist Assistant is responsible for providing occupational therapy treatments to patients to facilitate increased independence and functioning under the supervision of a Licensed Occupational Therapist.
Responsibilities include, but are not limited to:
- Implements patient's individualized treatment plan as established by the primary Occupational Therapist
- Work with other members of the rehabilitation team to implement programs consistent with the needs and capabilities of each patient
- Acts as an assistant to the Occupational Therapist when the Occupational Therapist is performing tests, evaluations, and complex procedures
- Instructs patients, families and other caregivers in the skills and techniques of the occupational therapy treatment program under the supervision of the Occupational Therapist
Requirements:
- Valid license to practice occupational therapy
- Skilled Nursing Facility experience preferred
Required Experience:
- Excellent observation, verbal and written communication skills, critical thinking skills, basic math skills.

We are currently seeking a committed and patient-focused Occupational Therapist to develop, recover or maintain the daily living and work skills of people with physical, mental, or developmental conditions. You will offer effective and client-centered services that will enable people to conduct everyday tasks or activities confidently and independently. The successful candidate will have a holistic perspective and will be able to adapt the environment to fit the person.
Responsibilities include, but are not limited to:
- Assessing, evaluating, and treating short-term and long-term residents that would benefit from skilled occupational therapy services
- Completing quality and timely documentation, such as evaluations, re-certifications, and discharges
- Directly and indirectly supervising occupational therapist assistants
- Conducting quarterly screens and/or evaluations on long-term residents
- Developing and assisting with overseeing restorative nursing programs
- Assisting with insurance authorizations and appeals
- Maintaining a clean and organized work environment
- Contributing to the overall needs of the facility and facility staff
Requirements:
