Job Purpose:
- As an LVN Wellness Coordinator at ARBOR HOUSE MASTER, your mission is to ensure our residents enjoy a fantastic quality of life. You'll be the go-to person for coordinating their care and keeping everything running smoothly.
Key Responsibilities:
- Work closely with residents and their families to make sure they have a great quality of life.
- Coordinate residents' care with physicians and third-party providers.
- Keep medications up-to-date in the EMAR system and ensure staff follows the orders.
- Monitor incident reports, as well as residents' weights and vital signs.
Required Education:
- Licensed Vocational Nurse (LVN) certification
- Experience in geriatric care
- Management experience
- A BIG heart
Required Experience:
- Experience working with residents and their families to enhance quality of life
- Experience coordinating care with physicians and third parties
- Familiarity with electronic medication administration record (EMAR) systems
- Experience monitoring incident reports, weights, and vital signs
Required Skills and Abilities:
- Strong communication and interpersonal skills
- Ability to ensure medication orders are followed by employees
- Detail-oriented with good organizational skills
- Compassionate and patient-focused approach
