Licensed Practical Nurse

Essential Duties and Responsibilities

The essential functions include, but are not limited to the following:

·        Provide skilled nursing care under the direct supervision of the clinical supervisor 

·        Complete evaluation tasks including vital signs and medication reviews 

·        Coordinate with physicians, therapists, and other individuals in the client’s care plan 

·        Assess and chart observations of the patient’s condition at each visit 

·        Maintain complete and accurate documentation of nursing care 

·        Work collaboratively with all members of the health care team 

·        Provide education to patients and families on proper home health care procedures and strategies 

·        Record patient’s medical information and vital signs   

·        Monitor, record, and report symptoms or changes in patient’s conditions   

·        Consult and coordinate with health care team members to assess, plan, implement, or evaluate patient care plans 

·        Administer local, inhalation, intravenous, or other anesthetics

·        Prepare patients for and assist with examinations or treatments 

·        Modify patient treatment plans as indicated by patient’s responses and conditions   

·        Order, interpret, and evaluate diagnostic tests to identify and assess patient’s condition 

·        Monitor all aspects of patient care, including diet and physical activity 

·        Assess the needs of individuals, families, or communities, including the assessment of the patient’s home or work environments, to identify potential health or safety problems 

·        Engage in research activities related to nursing 

·        Perform other duties as assigned 

Minimum Qualifications (Knowledge, Skills, and Abilities)

·        Associates degree required; Bachelor’s degree from school of nursing preferred 

·        Current and valid Licensed Practical Nurse license 

·        One year of LPN experience 

·        Current CPR certification required

·        Valid driver’s license required

·        Current automobile registration and insurance required 

·        Strong interpersonal skills that build personal rapport and establish trust with the patient

·        Computer literate 

·        Strong organizational skills  

·        Excellent written and communication skills 

·        Ability to meet physical demands of the job (ex: lifting 50 pounds for transfers) 

REQUIREMENTS

• Valid LPN license, Prior outpatient or clinic nursing experience preferred & Strong clinical and communication skills
