CASE MANAGER - Child & Family

Position Details 

Provide quality client centered case management services  that facilitate recovery and wellness for children & families  living with mental health / substance use concerns.   The Case Manager will support clinical treatment goals working as part of an interdisciplinary team. Positive outcomes are achieved through purposeful and respectful relationships rooted in trauma informed care, hope and evidence-based research.

Position Requirements

• Bachelor’s degree in Social work, Masters (in licensable field) preferred


• Experience working with children and families


• Experience providing services in-home and in  community based settings


• Valid Drivers license and clean driving record required.
