Pay: $22.00 – $28.00 per hour
Schedule: Monday–Friday, 8:00 AM–5:00 PM, with rotating on-call crisis coverage and some weekend availability

Position Summary:

Arbor Care Solutions is seeking a dedicated and compassionate Qualified Professional (QP) to join our Community Support Team (CST). This position provides direct rehabilitative and supportive services to adults with mental health and/or substance use disorders through community-based interventions.

The QP works as part of a multidisciplinary team to help individuals achieve recovery goals, improve daily functioning, reduce symptoms, and successfully integrate into their communities. This role requires flexibility, strong clinical judgment, and participation in a rotating 24/7 on-call crisis response schedule.

Reports To: CST Team Lead

Essential Duties and Responsibilities:

Direct Clinical & Rehabilitative Services:

  • Provide face-to-face services in community settings including homes, shelters, and public locations.
  • Deliver psychoeducation, skill-building, supportive counseling, and rehabilitative interventions.
  • Assist individuals in developing coping skills, emotional regulation, independent living skills, and recovery strategies.
  • Support relapse prevention planning and completion of functional assessments.
  • Maintain timely and accurate documentation in the electronic health record (EHR) in accordance with Medicaid and agency requirements.

Crisis Response & On-Call Coverage:

  • Participate in a rotating 24/7 on-call crisis response schedule.
  • Provide in-person crisis intervention services during mental health or substance use emergencies.
  • Collaborate with the CST Team Lead and treatment team to assess risk, stabilize situations, and coordinate appropriate care.
  • Assist with the development and implementation of crisis plans, WRAP plans, and psychiatric advance directives.

Care Coordination & Case Management:

  • Connect individuals with community resources, including housing, healthcare, employment, benefits, and legal services.
  • Assist with transitions from hospitals, residential treatment facilities, correctional settings, and other levels of care.
  • Coordinate services across multiple systems and ensure follow-through on referrals.
  • Advocate for individuals to support community integration and improved quality of life.

Treatment Planning & Documentation:

  • Participate in the development, implementation, and review of Person-Centered Plans (PCPs).
  • Ensure interventions are goal-oriented, measurable, and clinically appropriate.
  • Monitor progress toward treatment goals and communicate barriers to the treatment team.
  • Complete all required documentation within established timeframes.

Team Collaboration:

  • Attend weekly CST meetings, treatment reviews, and clinical staffing sessions.
  • Collaborate with Associate Professionals (APs), Paraprofessionals (PPs), Peer Support Specialists, and other team members.
  • Promote a team-based, person-centered approach to service delivery.

Quality Assurance & Compliance:

  • Adhere to all Medicaid regulations, agency policies, ethical standards, and clinical best practices.
  • Participate in quality improvement initiatives, chart audits, and ongoing training opportunities.
  • Ensure all services meet CST service definition requirements and documentation standards.

Qualifications:

Applicants Must Meet One of the Following Requirements:

  • Master's degree in a Human Services field and one (1) year of full-time post-graduate experience with individuals with mental health and/or substance use disorders; OR
  • Bachelor's degree in a Human Services field and two (2) years of post-degree experience with individuals with mental health and/or substance use disorders; OR
  • Bachelor's degree in a non-Human Services field and four (4) years of post-degree experience with individuals with mental health and/or substance use disorders.

Additional Requirements:

  • Minimum of one (1) year of experience working with adults diagnosed with mental health and/or substance use disorders.
  • Valid North Carolina driver's license.
  • Reliable transportation and proof of automobile insurance.
  • Strong communication, organizational, documentation, and problem-solving skills.
  • Ability to work independently in community-based settings.
  • Availability to participate in rotating on-call crisis response, including evenings and weekends.

Training Requirements:

  • Complete a minimum of 20 hours of CST-specific training within the first 90 days of employment.
  • Participate in required continuing education and ongoing professional development activities.
